During my earlier play throughs I always let him live. Didn't think about him as being a potential follower, just that my concious wouldn't let me. (even though it is only a game). I had trouble killing Sindig the wearwolf. So I tried it both ways. If I let him live then I had to kill many innocents. If I killed him, then he was the only casualty.

Let him live! I highly recommend it! As everyone else has stated thus far, he's a great follower. He's sneaky and great with a blade, but there's another huge perk: he's essential.

However on my second playthrough I spared him instead, because my Argonian assassin wouldn't kill someone who would be loyal to the Night Mother. If the Dragonborn chose to spare his life, Cicero is found upon exiting the Dawnstar Sanctuary after the quest where he becomes a possible follower. Cicero is flagged as essential before "The Cure for Madness". If he is spared, he becomes essential again, making him unable to be killed.
This may of been patched though.Usually I just kill him. Kind of a liability.

SPOILER somehow Cicero is effectively the only REAL Dark brotherhood member in the entire game, the others just took on their names and do more or less random assassinations, as they have no connection to Sithis or the Night Mother, which is IMO essential to being a member of the DB. So no, let him live.
Kill Cicero by simply attacking him with whatever weapon you have. Cicero only has around 300 to 500 health points, and he only wields a knife as a weapon so it's fairly quick (and easy) to kill him.
